“Having read the End of Life report, the recommendations align well to plans within Enhanced Primary and Community Care (EPCC) programme around End of Life (EoL) care and as part of the model of care for frail elderly and Long-term care (LTC) care, also with Clinical Commissioning Group (CCG) plans for commissioning more effective and compassionate end of life care services and support. 

Clearly there is much still to do, and discussions around key recommendations such as advanced care planning need to be turned into actions that deliver tangible benefits for our population. I will ensure that the report and its findings and recommendations are properly considered by the Enhanced Primary and Community Care (EPCC) programme.”
